Whether its under the ball of the foot, painful joints or shooting pain in your toes, this experience can make everyday activities surprisingly difficult. Walking, exercising, standing for long periods, or even wearing certain shoes can become uncomfortable and frustrating. Many people find themselves changing how they walk, avoiding activities they enjoy, or simply putting up with pain because they’re unsure what is causing it.
The good news is that forefoot pain is often very treatable once the underlying cause has been identified.
What Causes Forefoot Pain?
The forefoot contains a complex network of bones, joints, ligaments, nerves and soft tissues that work together to stabilise your foot and propel you forward when walking. When one part of this system is overloaded or irritated, pain starts and damage can occur.
Common causes include:
- Metatarsalgia (pain beneath the ball of the foot)
- Neuroma
- Joint inflammation or arthritis
- Stress injuries to the bones
- Bunions and toe deformities
- Overuse injuries from walking, running or sport
- Changes in foot function and biomechanics
- Reduced cushioning beneath the forefoot
- Poorly fitting or unsupportive footwea
Sometimes the problem is obvious, but often several factors contribute to the pain.

Our goal is to get you back to activity pain free, in a way that does not simply reduce your symptoms but finds the reason why they developed in the first place. That way we can prevent the problem happening over again and you can be confident in living life.
During your assessment, we take time to listen to your concerns, understand how the problem is affecting your life, and identify the factors contributing to your pain. We assess your foot structure, movement, footwear, activity levels and any underlying medical conditions that may be relevant.
This allows us to develop a treatment plan tailored specifically to you and your goals.
How We Can Help
Treatment depends on the cause of your symptoms but may include:
- Advice on footwear and activity modification
- Padding, strapping or offloading techniques
- Exercises to improve strength and mobility
- Custom or prefabricated orthoses
- Management of nerve irritation and inflammation
- Treatment of associated corns, calluses or skin lesions
- Guidance on returning to walking, exercise and sport
Where appropriate, we may also recommend further investigations or work alongside other healthcare professionals to ensure you receive the most effective care.
